In order to remove an item you have to select it by clicking or by using arrow keys on the keyboard and press the Remove toolbar button or Delete key on the keyboard. You can also achieve the same result by right-clicking the item you wish to remove and selecting Remove from the context menu. You cannot remove the item if it is the only placeholder available.
